Fortis Healthcare Profit Barely Budges as Expansion Costs Bite

Fortis posted a strong 17.5% revenue rise in Q1 FY27, but profit growth was a muted 2.3% as new hospital ramp-ups and costs squeezed margins.

Fortis Healthcare's Q1 revenue grew 17.5% on higher bed occupancy, but net profit margin fell 160 bps. New facility costs and ESOP charges held back earnings despite robust top-line expansion.

Fortis Healthcare’s first quarter was a story of two speeds. The hospital chain delivered robust revenue growth, powered by a significant expansion in its bed capacity. Yet, the profit line barely moved, revealing the cost of that very growth.

Revenue for the quarter ended June 2026 rose 17.5% year-on-year to ₹2,545 crore. Profit after tax, however, inched up just 2.3% to ₹273 crore. The net profit margin compressed to 10.7%, down 160 basis points from the same quarter last year.

Why the profit didn't follow revenue

Growth was driven by the hospital business, where revenue jumped 19% year-on-year to ₹2,187 crore, according to Moneycontrol. A 16.7% increase in occupied beds and a moderate rise in average revenue per occupied bed (ARPOB) fueled this expansion. The diagnostics arm, Agilus, grew a more modest 10.2%.

But this top-line strength was absorbed by rising costs. Newly acquired and commissioned hospitals like those in Manesar and Greater Noida are still ramping up, operating at lower margins. The company also booked a one-off ESOP charge of around ₹31 crore this quarter, notes the earnings call summary from Quartermark. Higher provisions for doubtful debts from government payers and legal costs added further pressure.

The margin bridge to FY28

Management has not backed away from its medium-term target. It reaffirmed the goal of achieving a 25% consolidated EBITDA margin by FY28, a figure that now includes the full impact of ESOP costs. The path relies on specific levers: newly opened units turning from a drag to a positive contributor, occupancy improvements, and normalization of the recent cost spikes.

For the current year, the expansion push continues. Fortis added about 100 beds in Q1 and plans to operationalize another 400 in the remaining quarters, including 200 at its flagship FMRI hospital in Gurugram once it receives an occupancy certificate.

Key Points

  • Revenue growth of 17.5% was led by a 19% surge in the hospital business, driven by a 16.7% increase in occupied beds.
  • Net profit margin fell 160 bps year-on-year to 10.7%, constrained by costs from new facility ramp-ups, an ESOP charge, and higher provisions.
  • Management maintained its FY28 target of a 25% EBITDA margin, banking on new units turning profitable and cost normalization.
  • The company plans to add roughly 500 beds in FY27, sustaining its capacity-led growth strategy.

Hexa Perspective

The quarter underscores the capital-intensive nature of hospital growth. Fortis is successfully adding scale, with occupied beds up sharply, but the earnings payoff is deferred. The unchanged FY28 margin target signals confidence, yet it is now a steeper climb, incorporating significant employee stock costs.

The next few quarters will test whether rising occupancy at new units can outpace the structural increase in expenses. For now, the story is clear: volume is arriving, but profitability is waiting for the facilities to mature.
